Before You See Us...

We ask that all patients have an appointment on the schedule when they are wishing to be seen.  Please contact the office at 402-421-3240 between the hours of 8:15 and 4:00 to schedule.  Please schedule using the reception line unless you need to be seen today then use your nurse line.  If you are requesting to become a patient at the clinic please use the new patient line. 

We ask that our new patients bring their completed forms, a photo ID, your insurance card(s), and your medication bottles with you to the appointment.  We want to make sure we are documenting the correct medication, dosage, and frequency.

If you are an established patient, we ask that you bring your current insurance card to each appointment.  We will ask to see your card when your insurance is showing as ineligible to make sure we have the correct information on file.  If you carry Medicare remember to bring your new Medicare card, the one that no longer has your Social Security number on it.

If you have lab orders from another provider that you would like drawn here, please give us a call first.  We don’t process all labs and sometimes it is better to go to a full lab to have the work done.

Consider activating your patient portal for easy access to your nurse between appointments.  Portal messages go straight to the inbox that they are viewing all day long, allowing for nursing staff to respond quicker.  Remember we do not interrupt patient care for phone calls unless it is an emergency (like sudden chest pain, shortness of breath, etc).  Need assistance setting it up your portal, give reception a call.

We ask all patients to arrive early for their appointments so we have plenty of time to get you ready for the doctor.  Established patients we request that you come 15 minutes early and for those new to the clinic we ask that you arrive 30 minutes prior to your appointment time.
